The Watchtower Society has produced an information pack for patients who are Jehovah's Witnesses undergoing surgery, they have included a band for the patients wrist [like those they give to babies] saying something along the lines of NO BLOOD

"No Blood" Dots

Among other "No Blood" indicators, we scatter the "dots" through a patient's chart; on the spine of the chart, demographic sheet, admission assessment, care map cover sheet, consent forms, blank anesthesiology assessment sheets, etc. Like Mark Eldredge said, the more places you can put indicators the more likely people will see them.

Ask your local HLC brother in America if they have them there for a definite answer,
I know Britain has them, if Britain has them, America probably does also.

Putting a noblood sticker on the front page of a patients notes may be a good idea.

I'm guessing doing the same for things like COPD, [not to give more than 35% O2] Do Not Resucitate [DNR], signed by 2 or 3 GPs etc and with patients agreement would be worthwhile also. [As well as clinical trial information, eg do not dispose of notes for 10 years etc]

Giving each of the GPs a score for how well they treat patients who are Jehovah's witnesses would be good [or how well they comply with bloodless medicine]
