Question Advanced Medical Directive sample form



Dear friends,
First of all congratulations for this wonderful site! What a great task is being done! Never give up mates!

I am new in this group and this is the first time i am posting a new thread, but i trust my question will be answered in the best possible manner. Thanks in advance folks!

I am trying to find the most approciate Advanced Medical Directive for refusing blood and/or blood derivates. I travel a lot so thinking in accidents etc so thinking of being very well prepared i wish to fill it up as soon as i can. Prevention is better than healing or as i say: 1 grame of prevention is better than 1 kilograme of healing.
So said that, i must add that after having done a research i could not find which one would be more suitable for my needs as i am spanish citizen, i reside in spain, and i should need one according to the laws and regulations from my homeland. Surfing your site i have found several drafts, for several states, mainly for the US, so i would appreciate very much if there is someone helping me out to find a correct model for me.

About the language, if the sample text is in spanish of course that would be perfect, but even if that is english that would be no problem to me as i travel to several foreign countries and it should be good to be able to carry with me in english.

Let me say that here in spain this is not so popular the bloodless medicine yet, and yes that is great that during the last decade it is developing very well and here everytime are more private clinics having more and more knowledge, and not being opposed to treat patients refusing blood transfusions, which is truly a blessing, but if we are talking about the social security system hospitals, that is still being another story. Day by day they are softer and more reasonable, and things are improving for better, but all depends the doctor you are having infront In general terms it is harder to find information, and i could not get anything for the document i need.
I readed amazed some of the posts here, stating than in America you can gain access to Advance Directives forms consulting your physician, hospital, governmental office on health care center, well there is not way here, only if they respect your wishes before/during/after a surgery is taking place, you are enough happy!

So anyway this is my this is my current situation, please help!!!!

The hospital I work for has their own medical directives and between that and the Society's I would like to think I am covered. Unfortunately that may not be the case. The integrity of the surgeon and anesthesiologist is always tested when faced with a crisis situation and some have been quite unkind in their failed cohersion attempts. I speak from experience within my family. The doctor flatly told us that my mother-in-law would die without blood. And I asked her what guarantees can she give us that she would live after receiving blood. Blood was administered but only at the insistence of my mother-in-law; she really had no knowledge, though not for the lack of trying on our part.
