Risks of electronic charts



My husband has recently been referred to a CHF cardiac specialist for medication control and monitoring. He was referred by an associate cardiologist who he sees for atrial fib.
For the past 8-10 weeks he has been seen every 2 weeks and has another appointment in another 2 weeks. He has had numerous changes in meds as well as several echos.
Our concern is that 2 visits ago, he was given an antibiotic for an upcoming dental procedure by this CHF Dr. However, upon arriving at the dental school, he was told that this was the wrong medication and not to take it.
When we went back to the CHF Dr., we told him he had prescribed an incorrect medication. He was very humbled, apologized and said, "you mean to tell me that I'm trying to keep him alive, and almost killed him!"

Our greatest fear in electronic charts almost happened. The physical charts clearly have a red allergy sticker on them and is hard to overlook. The electronic chart ???
On top of that, the last visit we had, the computers were down so the current changes weren't able to be accessed.

I know there is a place to get medical ID bracelets - I've heard of one that has a wrist compartment that holds the advanced medical directive - where can I get an order form?

we have a spreadsheet I devised listing all medication, doseage, prescribed by, prior surgeries, allergies, etc. and keep it with the DPA and our drivers licenses, but we need to do more and I thought this would be a good addition. Any help would be appreciated.

Wrist Bracelet with Medical Info

Hi Linda,

So sorry to hear of that close-call with medication mix-up. Electronic charting is still a controversial subject in the medical records world and the problems with that type of system are still being worked on, I'm sure. Of course, mistakes just like that can happen with a standard charting system as well. There are no perfect solutions in information management in this day and age, sadly. Not as long as the main element, imperfect humans, are involved...and they always will be in this current system we live in.

Anyway, as far as where to get a medical ID bracelet that has a compartment for a medical directive, I know that there are a couple of companies which sell wrist bracelets that have a compartment (I have one myself) but it's not big enough for the advanced medical directive itself...just a small paper that has the basic info on it and of course reference to finding the medical directive in one's purse, wallet, or whatever one wants to say.

The company from whom I purchased mine is www.findingking.com - just type in medical in the search box or see if there is a link in the left colum for medical items. I have also seen them for sale at www.drleonards.com and a couple other companies, so I'm sure you can find more if you do extensive searching.

There is always the option of getting a wrist wallet such as the kind runners or others use sometimes when working out to carry their ID. Those are big enough to fold up a medical directive and put it in there complete. They are not so attractive but they certainly serve the purpose.
