Thread: Looking for consent form

I'm trying to find a consent form for Jehova's Witness patients.

I used to have one that outlined what therapies and products they would and would not accept, but I don't have it anymore.

Can anyone point me to a consent form I can use, or model my own from?

The University of Iowa Hospitals and Clinics has a consent form specifically for Jehovah's Witnesses that I signed when having a procedure in the high-risk obstetrics unit there after a miscarriage. Women's Health Care (I was surprised after all the paperwork there was a separate "Jehovah's Witness patient" form just for us. It was nice to know my wishes would be very clear, though, in addition to my DPA & Living Will.)
