Blood Component Order Set?

Hello out there!
Our hospital is finally listening to me on developing a Blood Component Order Set to educate, encourage, monitor physician practice on blood transfusions. We have a few questions of those that may have in place Order Sets.
1) How do you get physician compliance on completing form? (My thought is that since it is blood components, that they do not leave the Blood Bank without the order set completed, seems easy enough.
2) Do you have independent physician practices, or are they employed BY the hospital system? Ours are independent, so it is a challenge with compliance.
3) Are you willing to share - fax, email, whatever, your Order Set so we can compare it to several and a draft that we have? What components do you have on form, criteria, indications, etc.???
4) Do you have other systems in place to monitor blood transfusions?
5) If you have an Order Set, is it carbon copied for different departments and the medical record? Idea to have it triplicated - Medical Record, Blood Bank, Blood Conservation ???
