We currently have a patient who has sickle cell disease, in vaso-occlusive crisis, who is one of Jehovah's Witnesses, and is refusing transfusion. Other that Hydroxyurea, fluids, and pain management, is there anything else we could do for transfusion alternatives? I'm assuming that EPO is a bad idea, in that it would produce more red cells with Hgb S. Thanks for your help.

I haven't seen a single patient with sickle cell disease in my several years of medical practice. Very rare in the Philippines, if ever there is one. I would suggest consultation with the experts in US hospitals.

bloodless surgery for child with sicklecell anemia
The child lives in England and has to have their tonsils & adenoids removed. The doctor wants to transfuse blood prior to operating. Are there any alternatives to blood transfusion. Been told EPO, Iron & Cell Saver arn't suitable. Please help.

There has been quite a bit of discussion about sickle cell disease in these forums. Go to the green menu bar at the top of this page, click on "Search" and type in "sickle cell". You will find much helpful information there. As a further suggestion, ask the treating physicians to contact the team at St. Christopher's Hospital for Children in Philadelphia, mentioned in the post by user Cj Tabbert above. Direct doctor-to-doctor conferencing often proves very helpful.

There are many adult Witnesses with sickle cell that have never had transfusions especially in Africa. There was a bloodless surgery done with a little girl (Not in the US) I don't know the methods though.
Regular blood transfusions can cause iron overload in these children which can lead to heart failure. I diet full of multivitamins and folic acid and drinking alot of water is important. Adults should drink four quarts a day children less and they should carry water bottles with them to school. |
