Managing HUNDREDS of blood clots

Is there anyone out there who has experience with managing HUNDREDS of blood clots in the femoral region? Patient is recovering from Guillain-Barre, has paralysis in bilateral lower extremities, had large blood clots that are breaking up into hundreds of smaller clots. Heparin has discontinued. Patient needs to move legs so more clots do not form.
