Preemie

How does one help a baby born at 27 weeks who had a PDA ligation? Baby is now 25 days old and platelets are continuing to drop from 92 to 66. Any suggestions for helping this child as the parents would prefer not to have a blood transfusion?

According to our clinical specialist in the NICU, it depends on the clinical picture of the infant. If the baby is stable and not actively bleeding, practitioners will allow platelets to drop to 50, even to 30, before transfusing. She stated that some kids just consume platelets, eg., if it's a chronic situation but they are otherwise healthy, no need to transfuse, yet.

Our NICU physicians allowed the platelet level to go to 8,000 before ordering a platelet transfusion.
