Jehovah's Witness/bloodless checklist?

I am in process of developing a checklist for patients to fill out to clearly delineate any "bloodless" wishes. I'm fairly certain this has been done before and don't wish to re-invent the wheel, so if anyone would like to share their checklist with me I'd be graciously accepting! ( I am a perfusionist who is tired of learning of the patient's wishes after the patient is asleep in the OR, and I hear secondhand from the anesthesiologist that "Oh, yea, he will accept this and that" with no further details!)
