I am a nurse that works for a physician's office. We have several patients that are jehovah's witnesses. Some of these patients are pregnant and some are gyn patients that we schedule for surgical procedures. After research I found this site and registered. I was wondering if you could help me with finding the correct forms for the patients to fill out.
