We have Blood Conservation as an initiative for the entire hospital. On our admission database we ask every patient 3 questions.
1. Do you have any concerns or questions regarding transfusion
2. Would you refuse blood for religious reasons
3. Would you refuse blood for personal reasons
An automatic consult comes to our office if the patient answers yes to any of these 3 questions.

Does anyone have an educational sheet that they give to patients that lists the risks of transfusion in lay terms and answer questions regarding tranfusions in general that they could share?
